Field Study Course Enrollment Form
The Field Study Course Enrollment Form is completed by students during
the quarter BEFORE they enroll in SE195 Field Study. The Field Study
Course Enrollment Form specifies the exact placement site where the
student will do field work, and provides a brief description of the
student's field responsibilities and goals. The Field Study Course Enrollment Form must be submitted online by the established deadline date.
PUBLIC HEALTH POLICY AND PUBLIC HEALTH SCIENCE MAJORS - Please use the Public Health Practicum Intent Form
Before the Field Study Course Enrollment and Public Health Practicum Enrollment Intent forms can be submitted, the student must complete the following:
1. Interview at prospective field study placement sites; and
2. Be accepted at the placement site listed below.
Note: Students are NOT permitted to change their field or practicum placement site after the Field Study Course Enrollment Form deadline.
